Greetings unto the Seneschalate of Atlantia, from the Kingdom Waiver
Secretary!

Based on some questions I have had come up at University, I wanted to
clarify two items regarding Minor Medical Authorization (MMA) forms for
minors attending events or practices in the care of someone not their
parent or legal guardian:

1) All minors unaccompanied by their parent/legal guardian must show up at
gate with a) a responsible adult, and b) a Minor Medical Authorization form
signed by the parent/guardian *AND NOTARIZED*, giving the responsible adult
permission to obtain medical treatment for the minor in case of emergency.
 The *AND NOTARIZED* part is not negotiable: if this has not been done, the
minor (very unfortunately) cannot be allowed to enter the site!

2) The MMA form, once checked, should be RETURNED TO the responsible adult,
not kept in the Troll/Gate files for the event/practice.  This is to
expedite matters in case medical treatment for the minor does need to be
obtained: no one wants to have to rush to Troll to dig frantically through
paperwork.  (It is additionally practical because signed, notarized MMAs
can be reused according to the parent/guardian's wishes, since the
expiration date can be placed as far in the future as desired.)

Please instruct your gate staff accordingly!
