Greetings unto the Kingdom from your friendly neighborhood Waiver Secretary!

I recently had some questions about the Minor Medical Authorization form
and when it needs to be notarized.  Accordingly, here is Kingdom policy on
the matter:

Any time that a minor (under 18 years of age) attends an event or official
practice without his or her parent/legal guardian, the adult responsible
for that minor MUST present a SIGNED *AND NOTARIZED* Minor Medical
Authorization form when checking in at gate.  If a signed AND NOTARIZED MMA
is not presented, the minor may not enter the site.

I highlight the NOTARIZED part because the Society website containing the
MMA forms is a little misleading - there is a separate and specific
combined Minor Waiver/MMA form for Trimaris (Florida), which, if read,
implies that a notarized MMA is only required in Florida.  This is
incorrect! Our fair Kingdom requires that ALL MMA forms be notarized
(because different legal jurisdictions have different rules about whether
or not a minor can be treated without notarized authorization from a parent
or guardian).

Please make sure to be prepared with a proper, notarized MMA if you are
escorting a minor for whom you are not the parent or legal guardian!
 Turning folks away at the gate is something that none of us wants to do.
