Quite frequent, my lady!  I gave Mordrea the mundane names of about a
dozen people, and at least half of the SCA names were misspelled (i.e.
not the registered form).  Remember that the reporting herald may not
have a chance to ask the person how they spell their names, and is
working from a 'best guess'.  Notice how many 'orphan' names there were:
'Atra'  'Caela' and nothing else.  Unless the individual herself (or
someone who knows her) gets back to Mordrea to say "My registered name
is _______" then Atra's AoA will never be attached to the file of
'Antonia da Lochmere'[1] as it should be. 

[1] a made up name, Mordrea!
