Actually, I've JUST set up a brand-new linkspage with examples of medieval 
appliqué -- including several examples with embroidered details on the 
appliqués, and a lot of examples showing different techniques for how the 
appliqué was stitched to the cloth.

It does look like doing the internal detailing first, and then cutting & 
attaching it to the background may be a better plan, but you may want to look at 
some of the different techniques used on the examples from the Middle Ages & 
Renaissance (ranging from Mamluk Egypt to Stuart England) to see if there's a 
style that more closely approximates what you want to do. :-)
