[MR] Questions used in the past for Service Animals

  The questions that have been used at Pennsic are this.


1) Are considered disabled under the ADA ? (Yes/No)

2) Does this Animal provide assistance  with that disability ? (Yes/No)


Notice we didn't care what the condition was or how the animal helps. 
Just to small questions.

Question 1)

I believe the reasoning went like this. If you are not "covered" under 
the ADA then the animal can NOT be a service animal covered by the ADA.

Question 2)

  You could still be covered by the ADA and NOT have a service animal. 
So we are asking is this a service animal.



While the wording has changed some over the years that is still pretty 
much the basic break down.
