According to the article in the Washington Post, she had an unknown condition that prevented her from clearing the active ingredient. So it built up to toxic levels over time. Only a blood test would have detected it, and as she was an extremely healthy athlete, didn't have a need for regular blood work. Reportedly, she did not use more than the recommended dosage at any one time.
