Eyes on a plate = St. Lucia, reflecting that by legend she plucked out her own eyes and gave them to an unwanted pagan suitor; but miraculously was able to see, though eyeless, for the rest of her life. 
 
A spiked wheel = St. Catherine, who was tortured on such a wheel. Sometimes the wheel is shown broken, reflecting the legend that the first time the torture was attempted, the wheel broke when it touched her flesh.
 
I'm not an expert across the board on this sort of symbology, but I know some of them.
