There are as many different answers to your question as there are knights.
As we gain wisdom through experience, our answers change.

Ask or Wait to be asked?  

Yes.

Some knights its one way, some knights its the other.

If the knight you are interested in learning from has other squires, other
household members, or just close friends you can talk to, that's always a
good way to go about it.
Sound them out.  Ask them what you just asked this list.  You'll be making
your journey in their company, might as well get to know them too. :)
