Um, how would he know the fellow was missing a kidney?  Did I miss
something?

Personally, anyone who could survive the doctors of the day for kidney
removal surgery was tough enough to handle anything!

(And I don't believe they had kidney removal surgery in the Middle Ages,
but I'm certainly no authority on the subject!)
