[MR] 60 bouts...a few questions

I lost the original post on this so please forgive if some of these questions were answered there...and a private response is fine by me.

First, do I have to be authorized?

Second, how do I document it?  Do I need their signatures?  Is there anything other than dates and names that I need to include?
