[MR] Another use for Minor Medical Authorization form (was RE: Waivers

My husband and I actually took the minor medical authorization form and
modified it for use by our parents to have on hand for when they are caring
for our children.  Just because they are the grandparents does not mean that
they can authorize any medical care for the children.  While hospitals and
doctors will do any *life-saving* procedures needed in an emergency,
understand that many things do not fall under that category.  Such as pain
relief, or even setting a broken bone immediately.

When the children have gone on trips with their grandparents we have a
provided a notarized copy for them to have.  While we have ever needed it,
it is a worthwhile insurance action to take.

Thanks for writing to confirm what forms you need to bring!  You and your
grandson will need the following two forms, which can be found at
http://seneschal.atlantia.sca.org/Waivers/:

- A Child Waiver, signed by his parent or guardian, and
- A Minor Medical Authorization form, which must be signed by parent or
guardian AND notarized.

(I am assuming that you are not the direct legal guardian of your
grandson-- if you are, the Child Waiver is all you need.)

More info about the Minor Medical Authorization can be found at

notarization is a nuisance, I know, but the form can be kept and used for
multiple events, especially as the parent or guardian executing it can
choose the date at which it expires.
