UPDATE!!

For a minor accompanied by an adult other than their parent or legal
guardian, a minor waiver (http://sca.org/docs/pdf/chldwaiv.pdf) signed by a
parent or legal guardian must be presented in addition to an original
signed and notarized medical authorization form for minors (
http://seneschal.atlantia.sca.org/Downloads/MMA_Form.pdf).
