Good Afternoon,
     I am a little confused here. I AM bringing a visiting nephew with me
this weekend and REALLY need the straight scoop before we arrive
(he's flying in from NY tomorrow and is REALLY excited about
attending Ymir). The two forms you posted in you email are the same
form, identical to the one called out for on the main SCA page (which
is what I have signed and notorized). Is there an additional form
that will be needed or are folks referring to the same form by two
different names (Medical Authorization & Minor Waiver)? Any guidance
would be appreciated (I'm running short on time before he gets here
and need to give my sister time to get the form notarized).
