You can also look at the Atlantian Web Site under Chancellor of Youth. In the section labelled parents, we have instructions on the forms you need, along with copies of each form. 

For anyone, please take a look at the Chancellor of Youth site, you never know what you could learn. Or if you have suggestions on items that should be added, then tell me!
