I recall being called into court once after undergoing knee surgery. I wasn't on crutches anymore, but my leg movements were stiff. Still, I was able to kneel...albeit clumsily...and kneel I did. However, being that they knew my leg wasn't functioning at 100%, Royalty was distressed that I had "caused myself discomfort" and voiced their concern gently to me. Frankly, I get the impression that they would have been far happier had I remained on my feet. They felt kind of bad about it, I think, although there was certainly no need. Had I felt unable to do it I assure you I would not have attempted it; but I wasn't in pain, just stiff and clumsy.
So no, no one need worry that anyone will think ill of someone who cannot kneel for whatever reason. Very often, a person is called into court to bestow some honor or recognition on them. The people bestowing the honor would not wish to see the object of their thanks inconvenienced, pained, or embarassed. That would pretty much defeat the whole purpose. ;-)
