I think there are a few reasons.
Some people have problems with their knees and cannot kneel without pain.
Some people know that they cannot get back up from a kneeling position
without holding on to something or someone and there is nothing to help them
in court.  Some maybe don't know that the pillow is for them, they may think
that it is for your feet to rest on.  And then there was that pillow on a
stand that almost made me tip over in court, not the best way to act in a
dignified way before Their Majesties.

Finally, I guess there may be a lack of information on how to conduct one
self at court.  My lord and I will be FOREVER grateful to one of our
Barony's gentles who gave a class to us newcomers on what happens in court
and how to behave.  I often see gentles going in front of Their Majesties
with a blade on their belt (which should not happen) and I find it sad when
a lady cannot find one person to lead her to court when she is called.
