[MR] Online Award Recommendation Tool Update

Howdy all.

Just made a minor update to the tool.   If you have lost your username 
you now can have it sent to the email address you registered with.

You can use your SCA name, modern name, or the email address you 
registered with to get the reminder sent to you.  There is a link off of 
the main page for this new function.

This is also a reminder to keep your profile up to date with your 
current email address.  You can edit your profile once you log in, just 
select the profile link <GRIN>.
