[MR] New Family Minor Waiver!

Greetings!

The SCA has recently approved the use of a new waiver form for families
with more than one minor.  This new waiver form has multiple lines on
which a family could list more than one minor on the same form, instead
of the old practice of needing to fill out multiple forms for multiple
children.  For at least one family I know with three minors, this could
save a lot of time!

One word of caution though: this waiver form can only be used for minors
from the same family.  It doesn't work like the adult roster waiver
form.  If you are bringing children to an SCA event from multiple
families, you will still need to bring separate minor waivers filled out
and signed by their parents or guardians for each minor.  Don't forget
you also need to bring Minor Medical Authorizations for minors attending
without their parents!

This new waiver form is called the "Family Waiver Form" and it is
