One teensy bit of clarification point to Mistress Mordrea's otherwise
lovely explanation of the various awards and orders:

As stated in Kingdom Law, "No kingdom award or order shall be
considered a prerequisite for any other kingdom award or order."  So,
while a lot of people do move up the ranks, as it were, by their
service, arts, or fighting, Their Majesties aren't limited by that.
Just because you aren't a companion of the Order of the Opal, for
example, doesn't mean that Their Majesties can't make you a Golden
Dolphin if They so choose (after consulting with that Order).

(Hmm, make that, "As will be stated in Kingdom Law".  See your April
Acorn for the shiny new Awards and Orders text, section 8 and Appendix
A of Kingdom Law.)
