[MR] Polling forms for Queens arms

Greetings,
    First let start by saying that this post, as stated in the subject line,
is not about the debate on Queen's device, good/bad.  My question is has the
form for voting been put on line yet?  I know there's one in the center of
the latest Acorn, but it's only one for a house of two people.  I don't
always have access to a copier or desire to pay for something that could be
printed in my own home.  Several people in my neck of the woods haven't
gotten their Acorn yet and would like a copy.
