There is a mailing list specifically for SCA glassworkers.  There are about
65 members as of today, and several of them are stained glass folks.  You
would be very welcome to join us!

The SCA glassworker's list includes cold glass (stained glass, mosaic work,
etching); warm glass (glass painting), and hot glass (lampworking, fusing,
glassblowing, enamel).  

In fact, this question just came up on the list (as in, about 3 days ago),
so let me forward the response on the list:

I have a book I like called A Thousand Years of Stained Glass, by Catherine
Brisac.  It is full of detailed illustrations and includes an analytical
text that focuses on the evolution of the various artistic styles, their
interaction with the then-current architectural modes, and how stained glass
design was influenced by other design of the period (in illumination,
enameling, etc.)

For technical details on how things are put together, though, you might try
Stained and Decorative Glass, by Elizabeth Morris, and Stained Glass: An
Illustrated History by Sarah Brown.  Morris states (p. 22) that the medieval
practice was to pour molten lead into boxes of reads to make it into thin
strips for glazing work, and that later the cames were cast in wood or iron
molds, and in the 16th C. the lead mill was invented.  Brown (relying on
Theophilus)(at p. 27) agrees with her that during the middle ages lead came
was cast in H-section molds by the glaziers themselves until the 16th C., at
which time lead mills came into use.

Brown also has a fascinating section on how cartooning was done, with a
picture of the only known remaining example of a medieval glazier's table,
complete with a picture of the 14th C. stained glass canopy made from that
same table.
