Well, I appreciate your offer, but these were not HTML pages, they were
documents in a Domino database and that requires a Domino server.  It
would take some non insignificant effort to render them out to a usable
format for hosting from a traditional HTTP server.  

You should contact Sir Edwin for the forms, he has all the materials
including electronic copies of the forms.  I am sure that he would be
happy to send a set to you.

I am also sure he can answer Galen's questions regarding the status of
any individual.
